How you register with us as a new patient
Details of the Practice registration area and how to register are available from staff at reception. But please note registrations will not be accepted over the telephone. Non UK residents will be required to bring their passports.
Registration forms can be obtained from the reception desk. Your medical notes from your previous surgery can take 6-8 weeks to be forwarded on to us from the Health Authority. Once the form has been completed, please give to the reception team to be processed.
Please ensure that the completed form is signed at the bottom of the first page and that you have included your NHS number which is a 10 digit number and you can obtain this from your previous surgery if you do not know what it is.
If you take any repeat medications please give the surgery a copy of your repeat order form.
